Physiotherapy for Lower Back Pain
Lower Back Pain Treatment
At SportsHealth Physio, our AHPRA registered physiotherapists provide same day appointments for lower back pain assessment and treatment. Whether your pain started this morning or has been building for years, we combine hands-on therapy, targeted exercise, dry needling, and shockwave therapy to help reduce pain and restore movement.
What Is Lower Back Pain?
Lower back pain refers to discomfort felt anywhere between the bottom of the ribs and the top of the legs. It may be a dull ache, a sharp catch when you move, or a band of tightness across the lumbar spine. It can appear suddenly after a single movement, or build gradually over weeks and months.
Around 90 percent of lower back pain is classified as mechanical or non-specific, meaning it relates to how the spine, muscles, and joints are moving and loading rather than to structural damage or serious disease. This is reassuring, because mechanical pain generally responds well to movement, education, and physiotherapy-led rehabilitation.
Pain is also influenced by sleep, stress, workload, and previous injury history, which is why two people with a similar scan can have very different experiences.
Lower Back Pain Symptoms To Look Out For
Presentations vary. You may notice one or several of the following:
Aching, stiffness, or a sharp catch across the lower back
Pain that worsens with sitting, standing, bending, or first thing in the morning
Difficulty changing position, such as getting out of a car or up from a chair
Muscle spasm or a feeling that the back has locked
Pain, numbness, or tingling travelling into the buttock, thigh, or calf
Reduced tolerance for walking, exercise, or a full day at work
Certain symptoms require immediate assessment at a hospital emergency department rather than a physiotherapy appointment. These include loss of bladder or bowel control, numbness through the saddle or groin area, severe or worsening leg weakness, fever or recent infection, unexplained weight loss, or back pain following major trauma such as a fall or collision. These signs are uncommon, but they matter.
How Physiotherapy May Help Lower Back Pain
Australian and international clinical guidelines recommend physiotherapy as a first-line approach for most lower back pain, ahead of imaging, injections, or surgery. Manual therapy, graded exercise, and patient education are consistently supported as effective components of care. Outcomes vary between individuals.
At SportsHealth Physio, treatment is tailored to your presentation and may include:
Hands-On Physiotherapy
Soft tissue therapy and joint mobilisation can help settle muscle spasm, improve lumbar mobility, and reduce pain, particularly in the early stages of an acute episode.
Dry Needling
Dry needling can help release deep muscle tension through the lumbar and gluteal region that may be contributing to your pain. Most of our clinicians are certified in dry needling and use it alongside other treatment.
Exercise Rehabilitation
Graded exercise is the most reliable way to reduce the risk of recurrence. Your physiotherapist prescribes a progressive program targeting core control, hip and trunk strength, and movement confidence, starting with something as simple as walking.
Shockwave Therapy
For persistent or chronic lower back pain, shockwave therapy may help reduce pain and support tissue recovery. Our certified physiotherapists complete a pre-treatment screening to assess whether it is clinically appropriate for you.
Education and Self-Management
Understanding what is happening in your back changes how you move and how you recover. We provide practical guidance on pacing, desk ergonomics, heat use, sleep positioning, and safe return to lifting or training.
What to Expect at Your Appointment
Your first appointment is built around understanding the cause of your pain and starting treatment the same day.
Detailed history: we discuss when the pain started, what aggravates and eases it, your work and activity demands, and any relevant imaging or medical history.
Physical assessment: your physiotherapist examines spinal movement, muscle strength, nerve function, and flexibility, and screens for red flags.
Treatment on the day: based on the findings, treatment begins in the same session, which may combine hands-on therapy, dry needling, or shockwave therapy with guided exercise.
Home program: you leave with a personalised exercise and management plan, plus clear advice on what to do over the next few days.
Same day appointments are available at both clinics. If your back pain started today, we aim to see you today.

Frequently Asked Questions

Most acute episodes improve substantially within two to six weeks, with the sharpest pain often easing in the first few days. Recovery depends on the underlying cause, how long symptoms have been present, and how consistently the exercise program is followed. Recurring or long standing pain generally takes longer and benefits from a structured rehabilitation plan. Your physiotherapist can give you a clearer timeframe after your initial assessment, and outcomes vary between individuals.
-
Gentle movement is generally preferred over prolonged bed rest. Lying still for long periods tends to increase stiffness and can make it harder to get going again. Short walks, comfortable range of movement, and prescribed exercises help maintain mobility and circulation. Activity may need to be modified rather than stopped altogether. If a particular movement sharply increases your symptoms, ease off it and raise it with your physiotherapist.
-
No. Physiotherapists in Australia are primary contact practitioners, so you can book directly without seeing a GP first. A referral or care plan is only required if you intend to claim through Medicare, WorkCover, TAC, or NDIS. Private health insurance extras cover generally does not require a referral. Our reception team can talk you through which pathway applies to your situation before you attend.
-
Most cases do not require imaging early on. Scans frequently show age-related changes in people with no pain at all, which means results can be misleading rather than helpful. Your physiotherapist conducts a clinical assessment to guide treatment and screen for red flags. If your symptoms are not progressing as expected, or if concerning signs are present, they may recommend imaging and refer you to your GP or a specialist.

Dry needling can help reduce deep muscle tension through the lumbar and gluteal region, which for some people contributes meaningfully to their pain and stiffness. It is used as one part of a broader plan rather than as a standalone treatment, and is typically combined with hands-on therapy and exercise. Most of our clinicians are certified in dry needling. Suitability is assessed before treatment, and responses differ from person to person.
-
Heat is generally more useful for the muscle spasm and stiffness that accompany most lower back episodes. A heat pack applied for ten to twenty minutes, with a cloth barrier to protect the skin, can make movement easier. Ice may suit some acute injuries where there is a clear traumatic cause. Neither resolves the underlying issue on its own, so pair them with gentle movement and physiotherapy guidance.
-
There is no single correct position. Choose whatever allows you to sleep, since sleep quality strongly influences pain. Many people find comfort lying on their side with a pillow between the knees, or on their back with a pillow supporting the knees. If mornings are consistently your worst time, mention it at your appointment, as it can point toward specific contributing factors worth addressing.
-
Yes. Long standing back pain responds to physiotherapy, though the approach differs from an acute episode. Rather than focusing on short-term pain relief, treatment emphasises graded exercise, building load tolerance, addressing contributing lifestyle factors, and rebuilding confidence in movement. Shockwave therapy may be considered where clinically appropriate. Progress is usually gradual, and realistic goals are set with you at the outset.
-
Recurrence is common, particularly when the contributing factors have not been addressed. Continuing your home exercise program, maintaining core and hip strength, managing workload spikes, and staying generally active all reduce the likelihood of future episodes. Early intervention when symptoms begin to reappear is often beneficial. Your physiotherapist will discuss a maintenance plan with you once your symptoms have settled.
