Enhanced Primary Care (EPC)
A chronic medical condition can be classified as one that has been present over six months with conditions such as cardiovascular disease, diabetes, musculoskeletal conditions (arthritis & spinal disorders) and stroke.
The Enhanced Primary Care Plan (EPC), now known as the Chronic Disease Management (CDM) allows General Practitioners (GP) to organise the health care of patients with chronic conditions. A GP can allocate a total of 5 sessions per person, per calendar year which Medicare will help fund.
Medicare offers a rebate ($52.95) for these treatments, however patients on care plans will need to pay the gap for the remaining consultation fees.
